About Your Visit

Referrals, insurance approvals and notes from referring physicians must be received in our office three (3) business days before your appointment.

At your first visit - please arrive 30 minutes before your appointment for registration and triage. Late patients may be asked to reschedule.

Please bring:

• valid, government issued photo ID
• insurance card(s)
• copayment (Cash, VISA, Mastercard only)
• complete and accurate list of all medications you are currently taking
• any CDs containing applicable imaging - X Rays, MRI, CT, etc.

Payment

We accept MasterCard, VISA or cash (no checks, please). Your co-pay and any outstanding balance are required at the time of your visit.
